Useful Forms

For your convenience many Student Association forms are available online. If you need a form which is not on this page, please visit the SA offices (SAC, second floor). Forms can also possibly be gotten by emailing sa-vp.

Senate Bill

Senate Bill Form: Word or PDF
Use this form to formally propose legislation in the SA senate. This form is also used to ask for money from the SA outside of the club budget process.

Club Forms

Club Charter Packet

Clubs are required to fill out a club charter packet every semester they are active. You may find club charter help useful. All of these forms need to be turned in before the budget deadline in order for a club budget to be considered.
