IRB Forms
IRB Application Form (pdf) (MS Word) -- Use this form to request IRB review of new projects, or existing projects that have not yet been reviewed by the IRB.
FORM A (Research Project Abstract) (pdf) (MS Word) -- You describe your project and its use of human participants on this form. Please fill out FORM A and attach it to the IRB Application Form.
Modification Request Form (pdf)(MS Word) -- Use this form to request a modification to your previously-approved research project.
Annual Renewal Form (pdf) (MS Word) -- Use this form to apply for an annual renewal of IRB approval for your multi-year project.
Adverse Event Reporting Form (pdf)(MS Word) -- Use this form to report unanticipated events or problems caused by or related to a human subject's participation in your research project. [ Adverse events are extremely unlikely given that NMT does not perform clinical research of any kind, but we make this form available as required by federal law.]

Online Training Courses for NMT Researchers
All PIs who perform human subjects research at New Mexico Tech are strongly urged to complete one or both of the following training courses. Federal law requires that all PIs be properly training in human research ethics before they start projects involving human subjects research.

- For Biological or Medical Research: The NIH Office of Extramural Research's Protecting Human Subject Research Participants
